Pebble Hills Band & Color Guard Absence Form

Please fill out this form anytime you have an absence outside of the school day, even if it has been excused through the school. Please be sure to fill out the correct date for the absence. This will help us maintain accurate records.

Please note that filling out this form does not mean the absence has automatically been approved. Just as when you miss school, you must provide specific details as to why you are absent and this will help us determine if your absence is excused.

Absences will be excused in accordance to SISD Policy and the Pebble Hills HS Band & Color Guard Handbook.
